Who We Are

Our Vision

UNYICN is an OASAS SOR-funded Regional Network Approach Learning Collaborative that brings physical, behavioral, and social care sectors into a conversation about regional and integrated healthcare delivery practices.

Our intention is to produce a collective impact on population health metrics for individuals and families through projects designed to promote equitable access and standardized best-practice care across our upstate New York regions. 

We are an open network where providers and friends of providers are welcome to attend, regardless of our various affiliations. We hope our participation in this collaboration will strengthen our regional healthcare footprint and position us as individual providers to attract more resources to produce the “value” in Value-Based Care.​

Our Mission

UNYICN’s SOR 4 Year 2 new funding cycle starts September 30, 2025.  We will collectively manage a budget of $2.5 million over the next year to continue building our community of providers in the Finger Lakes and Southern Tier Regions of New York State. We are focused on:

  • UNYICN is focused on integration, provider relations, and referral pathways, delivering care to persons and families holistically through an Equity Lens.
  • UNYICN is an Open Network that invites and includes providers and friends.
  • We have drawn attendance from physical, behavioral, and social care providers as well as four IPAs and two PPSs.
  • OASAS attends our meetings periodically.
