Who We Are

Our Mission
UNYICN’s SOR 4 Year 2 new funding cycle starts September 30, 2025. We will collectively manage a budget of $2.5 million over the next year to continue building our community of providers in the Finger Lakes and Southern Tier Regions of New York State. We are focused on:
- Building integrated pathways between Prevention, Treatment, Recovery, and Harm Reduction to promote whole person/family care
- Building integrated pathways between Health Systems, Behavioral Health, and Social Care sectors to achieve integrated healthcare delivery practices
- Designing and implementing regional projects and service delivery mechanisms to address our consumers’ unmet needs
- Operating an Integrated Care website, https://unyicn.org/, allowing providers and community members access to information about provider services so they can connect people to these services in real time.

- UNYICN is focused on integration, provider relations, and referral pathways, delivering care to persons and families holistically through an Equity Lens.
- UNYICN is an Open Network that invites and includes providers and friends.
- We have drawn attendance from physical, behavioral, and social care providers as well as four IPAs and two PPSs.
- OASAS attends our meetings periodically.
